Can AI Be Your Therapist? What AI Can and Cannot Do for Your Mental Health
People are talking to AI about their anxiety, their relationships, their loneliness. Some find it surprisingly helpful. Others come away feeling emptier than before.
As a therapist, I think it is worth looking at this honestly, not to dismiss it, and not to oversell it. AI is changing how people access mental health support, and that deserves a real conversation.
What AI actually does well
AI tools, whether chatbots, apps, or large language models, have gotten genuinely good at a few things in the mental health space.
- Providing information about mental health conditions, symptoms, and coping strategies in plain language
- Being available at 3am when you cannot sleep and need to process something out loud
- Offering a low-pressure space to articulate feelings before bringing them to a real person
- Helping with structured techniques like breathing exercises, journaling prompts, or thought records
- Reducing the stigma barrier, some people will talk to an AI before they will talk to anyone else
These are not small things. For someone who has never talked to a therapist, an AI conversation can be the first time they put words to something they have been carrying for years. That matters.

Where AI genuinely falls short
The limitations are just as real, and it is worth being clear about them.
AI does not have clinical judgment. It cannot assess risk. It cannot notice that the way you described your sleep has changed over the past three sessions, or that something in your voice sounds different today. It responds to the words you give it, not to what you are not saying.
- It cannot build a real relationship with you, and relationship is one of the strongest predictors of therapeutic outcomes
- It may validate things that should be gently challenged
- It has no memory of who you are across conversations unless you remind it
- It cannot sit with you in silence, or notice when you need to pause
- It is not equipped to handle crisis situations, trauma processing, or complex psychiatric presentations
There is also a subtler risk. AI is very good at making you feel heard in the moment. But feeling heard by a machine is not the same as being understood by another person who has their own history, their own limitations, and chooses to stay present with you anyway. That distinction is not trivial in therapy.

So can AI replace a therapist?
The honest answer is: not for most of what therapy actually does.
Therapy is not primarily about getting information or strategies, though those are part of it. It is about having a consistent, boundaried relationship with someone trained to help you understand yourself more clearly over time. That process requires a real human, someone who can be wrong, who notices things, who brings their training and their own humanness into the room.
Research on what makes therapy effective consistently points to the therapeutic relationship as one of the most important factors, more than the specific techniques used. AI cannot replicate that.
Where AI can fit in wisely
That said, dismissing AI entirely would miss something real. Used thoughtfully, it can be a useful complement, not a replacement.
- Between therapy sessions, to process something that came up
- As a first step for someone not yet ready to talk to a person
- For general psychoeducation, learning about what anxiety or burnout actually is
- To practice skills like identifying cognitive distortions or writing out a worry
- As a bridge when someone is on a waitlist and needs something in the meantime
The key word is complement. AI works best when it supports a human process, not when it substitutes for one.
A note on where this is going
AI in mental health is moving fast. Tools are getting more sophisticated, and the access question is real, therapy is expensive and not available to everyone. It would be naive to pretend AI has no role to play in that gap.
But access to something is not the same as access to the real thing. A chatbot that helps someone feel less alone at midnight is doing something valuable. It is just not therapy.
